- Your full name and NPI number
- The URL of your profile on this site
- A brief statement confirming you are the provider (or authorized representative)

Note: The underlying data comes from CMS's NPPES public registry. Removing your listing from this site does not affect your NPPES record. To update NPPES itself, use nppes.cms.hhs.gov.
